This notice describes how Jori Health obtains express written consent from patients before sending SMS messages, what those messages look like, and how a patient can stop receiving them at any time. It is published so patients, carriers, and regulators can verify our opt-in flow without requiring patient-portal access.

Jori Health sends transactional SMS messages to patients about their care — appointment reminders, pre-call confirmations, medication notifications, care-coordination updates, and patient-account notifications.

We send SMS messages only to patients who have given affirmative, SMS-specific consent through the Jori Health patient portal. Phone numbers collected for billing, scheduling, or other purposes are never opted in by default.
- 1.3
Patient consent is captured per-patient, timestamped, and version-stamped so any future change to the disclosure language does not invalidate prior consents.
02Where consent is captured
- 2.1
Patients consent to receive SMS messages inside the Jori Health patient portal at patient.jori.health, as a discrete step during account onboarding.
- 2.2
The opt-in step is a standalone, SMS-specific checkbox — it is notbundled into a general “I agree to communications” checkbox or hidden inside a Terms & Conditions acceptance.
- 2.3
The checkbox is unchecked by default. The patient must take an affirmative action to check it before clicking Continue.
- 2.4
Consent is not a condition of treatment, payment, or enrollment with Jori Health or any provider using the platform.
03Verbatim consent disclosure
The exact disclosure language shown next to the opt-in checkbox in the patient portal is reproduced below verbatim:
“I agree to receive text messages from Jori Health at the phone number on file. Approximately 20–50 messages per month. Msg & data rates may apply. Reply STOP to opt out or HELP for help at any time. Consent is not a condition of treatment.”

Directly above the checkbox, patients see a “What to expect” summary listing the categories of messages they will receive (appointment reminders & confirmations, lab results & care updates, medication refill notifications, transportation & scheduling coordination).

04What messages we send
All SMS messages are transactional and directly related to the patient's care. We do not send marketing or promotional SMS. Representative samples below.
“Jori Health: Your appointment with Dr. Smith is confirmed for June 3 at 2:00 PM. Reply STOP to opt out or HELP for help.”
“Jori Health: This is a reminder that your call with our care team is scheduled for tomorrow at 10:00 AM. Reply YES to confirm or RESCHEDULE to pick a new time. Reply STOP to opt out.”
“Jori Health: Your medication refill request has been submitted to your pharmacy. You will receive a notification when it is ready. Reply STOP to opt out.”
“Jori Health: Your care team has a quick question about your recent visit. Please reply at your convenience. Reply STOP to opt out.”
05Message frequency

Patients receive approximately 20–50 messages per month. Frequency varies with the patient's care needs — patients with active treatment plans may receive more frequent messages than patients in long-term follow-up.

Patients can opt out at any time using the methods described in Section 7.
06Message & data rates

Message and data rates may apply, depending on the patient's mobile carrier and plan. Jori Health does not charge patients for SMS messages.
07How to opt out

Reply
STOPto any SMS message from Jori Health at any time. The patient will receive a single confirmation message acknowledging the opt-out and will not receive further SMS messages. - 7.2
Patients may also update SMS preferences inside the Jori Health patient portal at patient.jori.health.
- 7.3
Opting out of SMS does not affect the patient's clinical care, account status, or ability to use the patient portal.

09Brand & sender identity
- 9.1
All SMS messages are sent by Jori Health on behalf of the licensed healthcare provider with whom the patient has an established treatment relationship. Every message begins with the literal text
“Jori Health:”so the sender is unambiguous. - 9.2
Jori Health does not share patient phone numbers, consent records, or message content with affiliates or third parties for marketing purposes.
10Mobile data sharing
- 10.1
Information collected during the SMS opt-in process — including the patient's mobile phone number, consent timestamp, and the version of the disclosure they accepted — will not be shared with third parties or affiliates for marketing purposes. This information is used solely to deliver the messages the patient has agreed to receive and to maintain auditable consent records.
11Changes to this notice

Jori Health may update this SMS consent notice from time to time. When the disclosure language changes, the version identifier in the page metadata changes with it, and existing patients are re-consented through the patient portal before any new disclosure language takes effect.
